Names on Home Deeds v. Mortgages?

When buying a home, does everyone signing or cosigning for a mortgage need to also be on the deed to a home?

I don't think it's smart but if your helping your child or something I don't see why this would be a problem. I'm not a banker though so maybe someone else on here could confirm this advice.
