Looks like the Prius is having brake problems.

I am guessing the cop who died spent way too much time hitting the 'keyless' off-switch - it has to pressed for 5 seconds before it will actually shut the engine down. So he was standing on his brakes, calling 911, pressing the off switch and may not have thought of neutral. Is there a possibility that there is a speed-interlock on the gear shifter? It was one of the fancier models that he was driving.

In addition to the Prius brake investigation, it appears that there may em interference with electronics from external sources. This is not a good thing and may mean that any Toyota buy could turn out to be a long-term bet. Toyota is a great company but with all the investigations going on, the price will probably drop and stay low until there is some resolution.

The hysteria is taking on a life of its own. Kinda like there is blood in the water - you don't see this ferver for any of the domestic brands with recalls out there.

Most likely there is a software issue. External sources of interferance are very low power. Don't know if any wiring is cars is shielded - sounds like that would be a good idea though.

Just to keep things in perspective - of the brake pedals - out of the millions made - only 8 had a problem. Out of the millions of Prius's built - 100 complaints - no accidents.
